Each SUNDAY game for U10 and older requires a game card. This game card lists your players for that game, is the official record for the game, and is required by the referees to start the game. Please download and complete the card to the right. This one sheet will print 3 copies of this card to cut apart. Save this document on your computer through the season. You can update the card's game information weekly. The referees will collect two copies of the card from you prior to the game. At the end of the game, the winning coach or the home coach (in the event of a tie) is given these cards to mail/fax into their division commissioners.
